![]() PAYMENT: Although we participate in several major health plans for general dermatology, cosmetic procedures and treatments are not covered by insurance. For these services, we request payment in full at the time of your visit. We accept MC, Visa, cash, and checks. INSURANCE: If you are a member of a managed care insurance company, please tell us when you make your appointment. Please be certain to provide the office with all of the required referral and authorization forms prior to, or at the time of, your visit. Without the necessary referral forms, payment for services rendered is due at the time of your appointment. For covered services, insurance co-payments are always due at the time of your visit. CANCELLATIONS: If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice. This courtesy, on your part, will make it possible to give your cancelled appointment to another patient. EMERGENCY AND AFTER HOURS CALLS: When the office is closed, one of our physicians is on call for true medical emergencies. We ask that you hold non-emergent matters such as prescription renewals and appointment scheduling until the next business day. PRIVACY: We take the issue of our patients' personal privacy very seriously. We pledge to never make available to any third party any personal, private, medical, and/or any other information. We will never give your email address to anyone. Health and medical websites have a particular responsibility to protect the privacy and confidentiality of their users. You have a right to use such sites with the confidence that your personal, private, medical, and/or any other information is not being tracked, collected, otherwise aggregated, or disclosed. |
